Job Description

Essential Functions

  • Attends work as scheduled and follows the community's call of procedures.

  • Maintains accurate and timely individualized service plans, document and communicate changes to the appropriate personnel.

  • Reviews service plans and other documentation to determine and /or promote appropriate services offered.

  • Schedules and participates in care conferences with families and residents.

  • Ensures appropriate service documents are completed by department personnel and compiled in an organized fashion for use in service plans and all care conferences.

  • In coordination with the appropriate department manager, makes non clinical service changes and collaboratively communicates changes to the appropriate personnel.

  • Assists the DRC in coordinating resident care assignments based on the residents' assessed needs.

  • Issues and compiles new resident and family surveys and distributes to appropriate personnel.

  • Coordinates new residents move-in process in collaboration with applicable Department Heads.

  • Completes new resident orientation with all the new residents.

  • Encourages residents to set individual goals and coordinates service plans to help them achieve their goals.

  • Is the liaison for residents/families, when necessary, to assist in service resolution, or direct to the appropriate personnel.

  • Ensures continued relationship with the residents that are in patients in the hospital and rehabilitation facilities through visitation or phone calls.

  • Maintain relevant documentation in an organized and secured manner.

Non Essential Functions

  • Attends all in services training and pre shift meanings

  • Maintains confidentiality of the residents.

  • Performs all duties in adherence to Spring Hills, LLC standards.

  • Maintains a positive and professional demeanor toward all residents, visitors, and co workers.

  • Adheres to all policies and procedures of Spring Hills, LLC

  • Performs other duties as assigned.

  • Oral comprehension - The ability to listen to and understand the information and ideas presented through spoken words or sentences.

  • Speech Recognition- The ability to identify and understand the speech of another person.

  • Speech Clarity - The ability to speak clearly so others can understand you.

  • Problem Sensitivity - The ability to tell when something is wrong or is likely to go wrong. It does not involve solving the problem, only recognizing there is a problem.

  • Written Comprehension - The ability to read and understand information and ideas presented in writing.

  • Inductive Reasoning - The ability to combine pieces of information to form general rules and conclusions ( includes finding a relationship among seemingly unrelated events)

  • Deductive Reasoning - The ability to apply general rules to specific problems to produce answers that make sense.

Job Requirements

  • Some College

  • One or more years in a hospitality and/or healthcare setting

  • Experience working with the geriatric population is a plus

  • Efficiency in computer software such as Microsoft Word, Excel and Power Point

  • Attentive to details and organized
