Job Description

This position is responsible for strategically growing the wellness services of the agencies home care division.  This will be done by representing the agency on various levels cultivating relationships and growing a client base.  Responsibilities include developing and maintaining a caseload of active wellness clients as well as assisting in the sales and marketing of the agencies wellness continuum to ensure continued growth.  This position will assist in creating community initiatives, which allow the agency to raise its public exposure.  In addition, this position will be responsible for performing wellness presentations and functional screens within the community as well as to other prospective clients and professionals in the industry. This position will be integral to the long-term vision of the agency and will be responsible for scaling the wellness continuum so it can be implemented across the companies national footprint.  Once the program is brought to scale this position will be integral to in the expansion of the program.  Reports to the Director of Home Care Services
Essential Functions


●       Works closely with the Director of Home Care Services as well as the Director of Business Development to ensure proper execution of the Spring Hills Home Care Wellness Program


●       Will develop and maintain a caseload of wellness clients within the Spring Hills community as well as in the surrounding community


●       Wellness clients will be evaluated using evidence based functional outcomes measures to track progress


●       Assessments will be done every 12 weeks to ensure the wellness client is making progress towards goals


●       Plans of care will be dictated by Health and Wellness Coordinator as well as the client/clients family/POA


●       Plans of care are open ended with emphasis on the client commiting to the wellness program long-term to ensure they remain functioning at their optimal potential


●       Records daily progress notes for every resident participating in wellness programming


●       Keeps master spreadsheet of all data on wellness clients


●       Provides monthly report that breaks down current clients on caseload as well as prospective clients and growth potential


●       Attends sales and marketing events to promote the Wellness program


●       Assists in developing relationships with professional referral sources and introduces wellness services to the public. (i.e. physicians, hospitals, senior centers, SNF’s, etc.)


●       Maintains close communitcation with community nursing staff to ensure an optimal plan of care for each wellness client


●       Increases and maintains wellness caseload goals. Converts prospects into clients.


●       Keeps detailed documentation on each wellness client.  Method of documentation TBD by VP of Home Care Operations


●       Assists in the hiring of future Exercise Physiologists as determined by growth needs


●       Once a steady caseload of wellness clients (exact number and visit total TBD) is a achieved, this position will start to expand program footprint to other regions of the company


●       Attends work as scheduled, including nights and weekends, and follows call-off procedures.

Job Requirements

Qualifications/Skills/Educational Requirements:
Master’s Degree in Clinical Exercise Physiology or related field as well as program development, sales, and marketing experience.
